I am the participant, or the parent/legal guardian of the minor listed below (“Participant”).

In consideration of being permitted to participate in Bridge Basketball Academy, LLC (“Bridge Basketball Academy”) programs, training, activities, camps, clinics, and use of its facilities, I (on behalf of myself, my spouse, my child/ward, and our heirs, assigns, personal representatives, and next of kin) agree to the following:

1. Assumption of Risk. I understand that participation in basketball training and related activities involves inherent risks, including but not limited to serious physical injury, illness (including exposure to communicable diseases), permanent disability, and death. I understand that while rules, equipment, and personal discipline may reduce risks, the risks cannot be eliminated.

2. Voluntary Participation. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of Bridge Basketball Academy or others, to the fullest extent permitted by law. I accept full responsibility for the Participant’s involvement.

3. Compliance. I agree that the Participant will comply with Bridge Basketball Academy’s rules, policies, and instructions. If I observe any unusual or significant concern regarding the Participant’s readiness or the activities themselves, I will remove the Participant from participation and immediately notify a Bridge Basketball Academy representative.

4. Release of Liability. I hereby release and hold harmless Bridge Basketball Academy, its owners, directors, officers, officials, employees, agents, volunteers, sponsors, advertisers, other participants, and if applicable, the owners/lessors of premises used to conduct the activities (“Released Parties”) from any and all claims, demands, causes of action, damages, losses, or liabilities of any kind arising out of or related to the Participant’s participation, including injury, illness, disability, death, or loss/damage to property, whether arising from the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

5. No Lawsuit / Indemnification. I agree that I will not make a claim against, sue, attach the property of, or prosecute any of the Released Parties for any claim arising from participation. I further agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, suits, costs, or losses arising out of or related to the Participant’s participation or use of the Bridge Basketball Academy facility.

6. Medical Fitness. I attest that the Participant is physically fit to participate and has no known medical conditions that would prohibit participation. I understand that Bridge Basketball Academy does not provide medical insurance or medical care.

7. Parent/Guardian Acknowledgment. As the parent/guardian, I confirm that I have explained to my child/ward:

(a) the risks of participation,

(b) their responsibility to follow rules and instructions, and
(c) that my child/ward understands this Agreement.

8. Equipment Responsibility. I understand and agree that I am solely responsible for the condition and safe operation of any equipment the Participant uses, including equipment obtained from Bridge Basketball Academy, and I agree to continuously inspect and maintain such equipment.

9. Photo/Video Release. I grant Bridge Basketball Academy permission to use the Participant’s name, image, likeness, and/or photographs/videos taken during activities for promotional purposes, social media, and public display, unless I provide written notice to opt out.

I HAVE CAREFULLY READ THIS RELEASE AND DISCLAIMER AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
